If you are an Existing patient please read the following information before moving forward to schedule your appointment with OC Chiro as it is vital to the appointment making process.
If you are a New Patient please go to the New Patient tab on our webpage to complete your New Patient paperwork and schedule your appointment (Your appointment requires more time)
1.) If you are an Existing Patient who HAS treated at OC Chiro within the last 3 months and DO NOT have a new accident or injury to report since your last visit, please Click on this Link it will take you to our secure online scheduler to make your appointment.
2.) If you are an Existing Patient who HAS treated at OC Chiro before but has not been seen in the last 3 months. Click this Link to fill out and submit an OUCH form. Once you have submitted the form you will be prompted to click on a link that will take you to our secure online scheduler to make your appointment.
3.) If you are an Existing Patient who HAS treated at OC Chiro within the last year and have a NEW accident or injury that you need to be seen/treated for. Click this Link to fill out and submit an OUCH form. Once you have submitted the form you will be prompted to click on a link that will take you to our secure online scheduler to make your appointment.
4.) If you are an existing patient who has NOT treated AT OC Chiro within the last year, please click you will need to fill out and submit New Patient paperwork. This will allow our doctor(s) to get re-acquainted with you and treat you in the most effective way possible. Lets get started, click this Link
