Insurance
We are Out-Of-Network (OON) with most insurance plans; however, many have OON benefits for chiropractic. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment. ***Note: Ultimately it is the responsibility of the patient to pay for services if the insurance company does not pay***
Cash Patient
For patients who would like to pay out of pocket or do not have chiropractic benefits, the following are our cash rates:
New Patient (30 min)
Dr. Hubbard and Dr. Gutierrez $170
Dr. Bell $130
Ongoing Active Release Technique/Chiropractic
Dr. Hubbard and Dr. Gutierrez $85
Dr. Bell $65
Returning Patient Re-Evaluation (for existing patients that haven't seen the doctor in 6 months or more)
Dr. Hubbard and Dr. Gutierrez $115
Dr. Bell $115
Credit Card
A credit card is now required at the time of scheduling. If you are unable to make your appointment, we ask you let us know 24 hrs (1 day) in advance to reschedule or cancel your appointment. Failure to do so will result in your account being fully charged the full cash rate for your appointment and it will not be covered by your insurance. This includes No Shows as well.
