**Due to the poor quality of insurance policies & practices, effective 7/8/2013 we will no longer be a preferred provider with Ohio Bureau of Worker's Compensation** What this means to you, our patients, is that we will no longer be able to bill OhBWC Managed Care Organizations (MCO). Dr. Dan is still able to treat you, but you will have to seek reimbursement from your MCO personally. CFH is happy to provide you with the necessary claim form at each visit to seek reimbursement.
