Step-by-Step

Nutrition Process Step-by-Step:

1. Patient prepays a non-refundable in office fee of $100.00 for a comprehensive nutrition evaluation and receives 7-Day Food Diary and other nutrition forms. 
2. At the time of payment, the patient schedules their first appointment 7-10 days in advance. 
3. Before coming in for the first appointment, patient fills out the Systems Survey Form, completes other nutrition forms, and records a 7-Day Food Diary so Dr. Kovacs can pinpoint any specific issues stemming from the patient's diet.
4. At the end of the first appointment, a second visit is scheduled for the following week.
5. The second visit is $55.00 and consists of a 20-30 minute nutritional report and recommendation meeting with Dr. Kovacs. At this visit, Dr. Kovacs will determine what supplements will enhance the patient's health. Supplements and herbs are priced separately and payment is due at time of service.  
6. Any future follow up visits cost $45.00 each and are scheduled as recommended by Dr. Kovacs.
