How to Prepare

What About Pre-Existing Medical Conditions?

Since you are operating such a large vehicle, safety is important. Some medications may interfere with that safety and disqualify you from obtaining your medical card. If you are on any medications that may interfere, if your Primary Care Provider is able to provide a letter stating the medication will not affect your ability to drive, you may still be able to receive your medical card. If you have any of the following medical conditions, documentation must be brought with you or submitted before your exam. Missing documents may impact your clearance.

  • Diabetes
  • Sleep Apnea
  • Heart Disease
  • Depression/Anxiety
  • History of Alcohol/Substance Abuse/Rehabilitation
  • ADD/ADHD
  • Medical Conditions treated with Coumadin or Warfarin
  • For example, Atrial fibrillation

If you have been diagnosed with a health condition and need forms from your PCP filled out, please have those done BEFORE coming to your exam. (i.e. diabetes mellitus form MCSA 5870, Vision Evaluation Report form MCSA 5871, Obstructive Sleep Apnea log 30 days, etc.)

How Long will My DOT Medical Card be Valid?

Regulations allow for medical cards can be issued for a maximum of 24 months. Shorter cards may be issued based on the Medical Examiner’s evaluation of your specific health conditions. This may be to allow time for you to bring documentation to from your Primary Care Provider or a treating specialist. A new DOT exam is always required to issue a new medical certificate, regardless of its duration.

In some cases, the Medical Examiner may place your exam in official Pending Status. This basically pauses your exam for up to 45 days for you to return with health documentation. Pending status does not issue a medical certificate. You must return to the same clinic to complete your exam.

The cost of the DOT physical exam at Holistic Health & Chiropractic of Frankfort is $100 and can be scheduled by calling the office at (815) 469-7472.
