IF YOU ARE NOT ABLE TO DOWNLOAD THE APPROPRIATE FORMS, PLEASE REPORT TO OUR OFFICE 20 MINUTES PRIOR TO YOUR APPOINTMENT SO THAT YOU WILL HAVE TIME TO FILL THEM OUT BEFORE YOUR SCHEDULED TIME WITH THE DOCTOR.
