TBCE Proposes Amendment to Change of Address Notifications

The Texas Board of Chiropractic Examiners (Board) proposes amendments to 22 TAC §75.1 (Notification and Change of Address) to clarify contact information a licensee provides to the Board, and to make the deadlines for submitting changes to that contact information uniform. 

If adopted as proposed, the new rule would read:

§75.1.Notification of Change of Address, Electronic Mail Address, and Telephone Number.

(a) A licensee shall maintain a current physical home and business address with the Board. Additional mailing addresses may be provided. A licensee shall notify the Board in writing of a change within 30 days. The notification shall clearly identify the licensee, the address to be changed, the license number.

(b) A licensee shall maintain a current telephone number with the Board. A licensee shall notify the Board in writing of a change within 30 days. The notification must identify the licensee and include the license number.

(c) A licensee shall maintain a current electronic mail address with the Board. A licensee shall notify the Board in writing of change of electronic mail address within 30 days. The notification must identify the licensee and include the license number.

The proposed amendment is detailed below with added language in bold and deleted language in [brackets and italics]:

§75.1.Notification of [and] Change of Address, Electronic Mail Address, and Telephone Number.

(a) A licensee [Licensees] shall maintain a current physical home and business address with the Board. Additional [In addition, a different] mailing addresses [address] may be provided. A [beyond that of the home and business address. Within 30 days of a change in any of these addresses, a] licensee shall notify the Board in writing of a [the] change within 30 days [in writing via U.S. mail, electronic-mail, facsimile, or other written communication]. The notification [of address change] shall clearly [and legibly] identify the licensee, the address to be changed, the license number [number(s)] associated with the address, and shall be signed [by the licensee(s)].

(b) A licensee shall maintain a current telephone number with the Board. A licensee shall notify the Board in writing of a change within 30 days. The notification must identify the licensee and include the license number.

(c) A licensee shall maintain a current electronic mail address with the Board [board]. A licensee shall notify the Board in writing [board, by electronic mail,] of [any] change of [in the] electronic mail [mailing] address within 30 [five (5) business] days [of the change]. The notification must identify the licensee and include the license number.
