Welcome to the All About Wellness new patient center!
We understand that arriving at a doctors office for the first time can be arduous. Filling out paperwork, learning new faces and names, and getting to know your way around the office can be an overwhelming experience. For that reason, Dr. Neary and his staff at All About Wellness Chiropractic Center have provided an area within our website for new patients to obtain some of the necessary forms that they can bring to their first office visit. We want you to arrive as relaxed as possible!
By scrolling down on this page, you will find valuable information, and be able to enter online or print out our administrative paperwork. If you choose to fill out your paperwork on the day of your first visit with us, PLEASE ARRIVE AT LEAST 15 MINUTES EARLIER THAN YOUR APPOINTMENT TIME, so that we may see you and our other patients on time. Please be sure to bring a valid Driver's License and insurance card with you.
If you have seen another Chiropractor, or if you have been referred to us by any other physician, call his or her office and request a copy of your records to be faxed to us at (770) 864-9628. If you have had x-rays or an MRI performed recently, please bring a copy of those films, so Dr. Neary can review them during your first visit.
