Appointment Request

Dr. Curry's Patients ONLY may schedule an appointment online using our online scheduling app.  This appointment will be confirmed and will be automatically added to our schedule.  For other providers, skip to the form in the blue box at the bottom of the screen to request a time.

If you wish to schedule with one of our providers you will need to request an appointment time below or call our office at 937-848-8500 to schedule.  If you request an appointment via the form below our office will call you back to confirm an appointment time as close as possible to the one requested.  While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us!
