That is why at Associated Chiropractic Physicians our objective is to remove as many stressors causing unnecessary tension and we wish to make you feel as comfortable as possible.  In this area of the website you will be able to take care of a few key items that are necessary prior to arrival for your visit in the comfort of your own home.

You may have health insurance that would like to use to support your care in our office. To help navigate your health insurance questions we encourage you to visit our "Insurance/Payment options" and "Understanding Your Health Insurance" links.  Additional forms may be necessary and we will provide these for you in the office.
